Is the medicine prescribed for you listed in the formulary as a special authorization benefit or prescribed for a different purpose than that listed on the formulary?

If the medicine is listed, but has published criteria associated with it, then the prescriber must ensure that the criteria apply to your situation.

If there are no specified criteria or the medicine is prescribed for a different purpose than that listed on the site, then your prescriber must prepare and submit a request to the NL Pharmaceutical Services Division for review and approval.
